Overview

The Bilingual Child and Family Team (CFT) Facilitator's primary responsibility is to facilitate Child and Family Planning Meetings using wraparound principles that are family-centered, individualized, strengths-based, and outcome-focused. The program's goal is to help children and families with complex and enduring needs receive intensive treatment in the least restrictive environment that is appropriate. The Facilitator's role is tor pvide assessment of the client and family's needs and lead the team in providing treatment to support the family toward meeting their goals.

Responsibilities

  • Facilitation of Child and Family Planning Meetings. This includes:
    • Preparing for the meeting
    • Coordinating attendance
    • Facilitating the meeting
    • Writing meeting notes and distributing them within two days
    • Writing mental health notes
    • Cleaning up after the meeting
  • Case management and coordination of care
  • Collaborate with county and community partners including but not limited to county social workers, probation officers, outside therapists, psychiatrists, teachers, etc.
  • Complete mental health notes, Family Team Meeting notes, Wrap Action Plan, incident reports, and other documentation as necessary in a thorough, accurate, and timely manner
  • Intervene with clients' behavior management programs and provide crisis management as needed
    • In circumstances where there is serious threat of harm, this may include physical management/restraint of the clients (implementation of Seneca's Equilibrium model)
  • Maintain accurate documentation and paperwork including, but not limited to, time sheets, flex funds, and mileage
  • Attend team meetings, weekly trainings, individual supervision, and group supervision
  • Adhere to clinical best practices
  • Maintain a flexible schedule that includes early mornings, evenings, and occasional weekends
  • Participate in providing emergency on-call response as scheduled
  • Function as a program leader and uphold the ideals of the program
  • Utilize crisis communication and de-escalation techniques as per Seneca's Equilibrium model training. This may include intervening with the clients' behavioral program through physical management of the clients, in applicable. Training in additional crisis intervention modalities such as CPI may be required
  • Demonstrate professional behavior at all times in accordance with Seneca's mission and philosophy

Qualifications

  • Bachelor's degree
  • Bilingual in Spanish/English required 
  • 2 years' experience working with children/youth and families with complex and enduring needs in a mental health setting
  • Experience implementing family-focused wraparound services preferred
  • Ability to effectively work with a diverse population and workforce
  • Strong organizational, time management, and writing skills
  • Ability to work effectively as part of a multidisciplinary team
  • At least 21 years of age
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
  • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies' insurance policy
  • Own vehicle and current automobile insurance
  • Proof of COVID-19 vaccination or an HR-approved medical/religious exemption

Schedule

  • Full-time
    • Monday - Friday
    • 9:00 AM - 5:30 PM
