The TAY Clinician provides family-based mental health counseling, therapy, crisis intervention, family finding, and case management services to transitional age youth and families. TAY Clinicians are members of a treatment team and provide services at the program site and in the community, including, but not limited to hospitals, schools, and family homes. The TAY clinician works with clients ranging the ages of 18- 25 years of age. You will be traveling to meet with clients and ensuring they are connected to resources. This is a team based setting and will be working with a case manager. We work collaboratively with the Department of Public Health and with other community agencies within the Tay System of Care.
