Become a pivotal part of our team as a Social Worker for our Child Abuse Prevention Services (CAPS) Wraparound services and join us in providing family-centered and strength-based services!
As a Social Worker for Wraparound Services, you will provide culturally responsive services and leads a collaborative planning process for youth and families who are referred through Child Welfare, Juvenile Probation, or County Behavioral Health departments. Services will be provided almost exclusively in the community: in families' homes, foster homes, schools, other community locations, etc. for youth who are at-risk of being placed in out-of-home care or are stepping down from a higher level of care. Your primary role is to ensure a sustainable network of unpaid supports (i.e. extended family, mentors, teachers, coaches) are working together toward stability for the youth and family.
ABOUT THE PROGRAM
The Sonoma County Child Abuse Prevention Services (CAPS) Wraparound Program is an intensive home and community-based service offered to youth and families identified by Child Welfare as high risk for entering the dependency system. Service objectives include safety planning and stabilization of the home environment via community resourcing, short term youth and parent skills coaching, engagement + activation of natural supports, and referrals to other services/therapeutic treatment as needed to increase safety and divert youth from ever entering the foster care system. Services duration is 1-3 months for approximately 7-9 youth on average.
