Are you passionate about nurturing and supporting children and families in need? Do you have a strong desire to create positive change and help others thrive? If you’re driven by empathy, compassion, and a deep commitment to making a meaningful impact in the lives of others, join us in making a difference: Become a Care Coordinator for Our WISe Program.
As a Care Coordinator for our WISe Program in South Seattle, Tukwila and South King County, you’ll have the opportunity to make a profound impact on the lives of children and families by guiding them toward stability and well-being. You will work closely with families to develop and implement personalized care plans, always ensuring that their voices are heard and their needs are prioritized. By collaborating with a multidisciplinary team of professionals, you’ll address the various aspects of a child’s and family’s life, from their home environment to schools and community settings, making sure they receive the support they need to thrive. Your services will be delivered where families feel most comfortable—whether in their homes, foster homes, or schools—offering direct, in-home support that fosters stability and growth. Acting as a central point of communication, you’ll be a lifeline for families, helping them navigate difficult transitions with consistency, clear communication, and a constant focus on their well-being. Your dedication will ensure that each family has the resources, guidance, and care they need to overcome challenges and move toward a brighter, more stable future.
