We are hiring a Care Coordinator for our Wraparound Intensive Services (WISe) Program to ensure children and families receive culturally responsive mental health services. Now offering $500 sign-on Bonus   The Care Coordinator for our WISe Program coordinates services and planning for children and families referred and screened into the WISe program in Tacoma, Tukwila and Pierce Countyareas. As a Care Coordinator, you will guide, develop, implement, and monitor individual case plans and Child Family Team (CFT) to ensure client and family voice. This role will guide their team through the WISe process and phases and hold the central point of communication between the WISe team and the client. Services will be provided almost exclusively in the community: in families’ homes, foster homes, schools, etc. The primary role is to ensure that all parties work together toward stability for the child and family. Multisystemic Therapy (MST) is an evidence-based practice aimed at reducing behaviors like substance use, school truancy/failure and parent/child relationships. Are you an experience mental health professional with four years of experience and ready to take the next step in your career? Join Seneca's team in providing family-centered and strength-based services!   As a Care Coodinator of Wraparouns Services, you will provide culturally responsive services and leads a collaborative planning process for youth and families who are referred through Child Welfare, Juvenile Probation, or County Behavioral Health departments. Services will be provided almost exclusively in the community: in families' homes, foster homes, schools, other community locations, etc. for youth who could required being placed in out-of-home care or are stepping down from a higher level of care. Your primary role is to ensure a sustainable network of unpaid supports (i.e. extended family, mentors, teachers, coaches) are working together toward stability for the youth and family.   ABOUT PROGRAM Seneca offers "Wraparound" services to children and youth susceptible of placement, and to their families. The Wraparound programs are strengths-based, family-driven, flexible, and creative, with the goal of helping the family develop the skills and supports to prevent or reduce the possibility of out-of-home placement of their child. A family-based team addresses the needs of the entire family, not just the child of focus. Seneca's First Stop Visitation Center in Santa Clara provides services for children who have been removed from their family and who are working toward reunification. The visitation center is a resource for families to access transportation and therapeutic support in the reunification profess during visits at the center.    Bilingual Visitation Counselors observe family interactions and provide role modeling, coaching and verbal communication to ensure a safe and successful visit. This program is designed to provide opportunities for parents to build parental capacity, strengthen family relationships and increase successful reunification and permanency outcomes.     "I absolutely love my job. Join our team of mental health professionals at Kaiser Wrap today!   Seneca offers weekly individual and group supervision towards licensure hours, free CEUs, free trainings to improve clinical skills, and yearly scholarship opportunities for professional development.   The Social Worker will meet with clients and caregivers on a weekly basis to further treatment goal, provide psychoeducation, and coordinate care between providers. As the Social Worker you will have the opportunity to provide individual/family therapy and assist team members on casework issues. Services for families will take place in-person within the community (Petaluma) while other work can be completed remotely.   Candidates who are bilingual in Spanish/English are encouraged to apply.     ABOUT KAISER WRAPAROUND This Kaiser Wraparound Program is a partnership between Kaiser Permanente and Seneca Family of Agencies, providing behaviorally focused mental health services to children and families.
