The Wraparound Care Coordinator, is responsible for both the clinical treatment and case management of six to eight youth and their families who are referred through social service, and probation. The Wraparound Care Coordinator will act in close partnership and a liaison with Department of Social Services-Child Welfare Services, Juvenile Probation, Behavioral Health, and other service providers providing strengths-based and outcome-oriented alternatives to out of home placements for the youth. Our Care Coordinator is part of a multidisciplinary team to support the youth in meeting all treatment goals to ensure permanency and stabilization in current placement while increasing lifetime connections with others in the community and family. Treatment and support services are tailored to address behaviors and meet family needs, even as they change over time.
ABOUT WRAPAROUND
Seneca offers intensive "Wraparound" services to children and youth at risk of placement in a high-level residential treatments or juvenile hall, and to their families. The Wraparound programs are strengths-based, family-driven, flexible, and creative, with the goal of helping the family develop the skills and supports to prevent or reduce the possibility of residential placement of their child. A family-based team addresses the needs of the entire family, not just the at-risk child. This family-based team consists of family members, Seneca's Care Coordinators, Youth counselors, Parent Partners, Permanency Specialist and other community members who have an important role in the family,
