Now a $2,000 sign-on bonus for new employees!   As a Social Worker - Care Coordinator, you will coordinate and plan services for children who are referred through social service, probation or mental health departments and are in the process of stepping down from institutional into family or family-like settings. Services will be provided almost exclusively in the community in families’ homes, group homes, foster homes, schools, etc. The primary role of the Social Worker is to ensure that all parties are working together toward stability for the child and family. The primary responsibility of the Partial Hospitalization Program (PHP) Therapist is to provide clinical services to PHP clients, co-facilitate training for the PHP team, complete documentation, and support the clinical integrity of the program. Therapists in the PHP provide intensive, short-term stabilization services to enable clients to step down to lower levels of outpatient mental health treatment. The main treatment modality is group therapy.   About STEP Partial Hospitalization Programs The STEP (Skills, Transitions, Exploration and Progress (STEP) Program is a partnership between Kaiser Permanente and Seneca Family of Agencies. STEP provides intensive, short-term and stabilization services enabling youth to reintegrate into school and the community with lower levels of outpatient mental health treatment. The Foster Youth Social Worker is responsible for both the clinical treatment and case management of four to eight youth placed in intensive Resource Family Approved (RFA) homes, or a combination of these youth and lower needs youth placed in foster or adoptive homes. The Foster Youth Social Worker may also hold a combination of supporting caregiver in a "therapeutic foster care" role, where in the parent is a direct provider of mental health interventions and the clinician acts as their supervisor and coach, supporting their intervention development and overseeing daily mental health notes. The Foster Youth Social Worker will support resource parents in meeting all expectations to gain and maintain RF approval as well as needing the needs of the youth. They support the youth through clinical assessment and direct care to meet their immediate and long-term needs. They work as part of a multidisciplinary team to support the youth meeting all treatment goals and transition to a more permanent placement.   Seneca offers weekly individual and group supervision toward licensure hours, free CEUs, many free trainings to improve clinical skills, and yearly scholarship opportunities for professional development.   ABOUT PLACEMENT Seneca's Placement Programs operate with a core belief that every child deserves a family. Families are recruited, trained, and certified to provide permanency and safety for children of all ages who are not able to remain with their family of origin. Some are impacted by behavioral, emotional, developmental, or medical challenges and many have experienced trauma and loss. The program provides youth and children with the safety, stability, and support of a nurturing and trauma-informed family.
